Job description
Aveanna Healthcare is expanding its home health PT program in Kent County, Delaware, and seeks a Physical Therapist to join our patients’ care team. This role focuses on evaluating mobility, designing goals, and delivering therapy in patients' homes on a weekly, part-time schedule.
You will perform initial assessments, document progress, collaborate with physicians and caregivers, and support discharge planning.
