Job description
Bridge Home Health in Sacramento, CA is seeking a licensed Physical Therapist for home health services on a PRN basis. You will assess patients, develop and implement plans of care, and coordinate with families and physicians to achieve rehab goals.
The role requires CAPTE-approved DPT, active California license, CPR certification, and a valid driver's license. You will supervise PT assistants as applicable and ensure compliance with state practice act and organizational policies.
