Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to provide in-home services and help patients regain strength and independence under the Clinical Manager. You will plan and administer prescribed treatments in patients' homes to achieve the highest possible physical function.
You will assess progress, develop rehabilitation plans aligned with physician goals, supervise PTAs and aides, and document care thoroughly for physicians and families.
