Job description
Crossroads Home Health and Hospice is seeking a skilled Physical Therapist to provide in-home patient care across Contra Costa County. The role focuses on evaluating patients, designing individualized treatment plans, and delivering hands-on therapy in patients’ homes to improve strength and independence.
Candidates should have a current PT license with at least 1 year in home health, excellent communication, and the ability to work autonomously and within a team.
