Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in the comfort of their homes, reporting to the Clinical Manager.
You will plan and administer prescribed therapy, assess progress, guide goals, supervise PTAs and Home Health Aides, and document evaluations, plans and discharge summaries to support coordinated care.
