Job description
CenterWell Home Health in Mansfield, OH is seeking a Home Health Physical Therapist to deliver in-home therapy, assess patients and develop personalized rehabilitation plans to restore function and independence.
You will collaborate with clinicians, document progress, and support families. A valid PT license and CPR certification are required; one year of experience is preferred. This is a full-time, 40-hour-per-week role with competitive pay and benefits.
