Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home care, guiding patients to regain strength, mobility, and independence. You will work under the Clinical Manager and collaborate with a multi-disciplinary team to optimize functional outcomes in patients’ homes.
