Job description
CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home care, helping patients regain strength and independence. Reporting to the Clinical Manager, you will assess function, plan interventions, and coordinate with a multidisciplinary team to optimize outcomes.
You will supervise PTAs and Home Health Aides, document progress, and ensure treatment plans align with physician orders while maintaining high-quality patient-centered care in home settings.
