Job description
CenterWell Home Health in Clarksville, TN seeks a licensed Physical Therapist to join our home-based care team. Plan and deliver prescribed PT treatment in patients' homes, focusing on functional improvement and independence.
Collaborate with physicians, nurses and aides; document progress and adjust plans; provide clear instructions to PTAs and Home Health Aides. Full-time, 40 hours per week, on-site home visits.
