Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence from the comfort of their homes. Reporting to the Clinical Manager, you will deliver personalized care focused on functional improvement.
The role involves planning and administering PT, collaborating with a care team, documenting progress, and adhering to physician plans of treatment. Hours: 40 per week; pay per visit $59.00–$83.00; annual salary range $93,000–$128,000.
