Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their homes, reporting to the Clinical Manager. You will analyze and advance functional mobility, delivering personalized PT plans to improve safety and quality of life.
Required: unrestricted PT license and CPR; collaborative care with a multidisciplinary team; 40-hour work week with a base pay structure and driver safety considerations. Humana/CenterWell benefits apply.
