Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes. You will plan and administer prescribed PT treatment, assess patient progress, and collaborate with a care team to meet rehabilitation goals.
As part of Humana’s CenterWell, you will document interventions, coordinate with physicians and family, and provide clear care instructions to PTAs and aides.
