Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes. You will plan and administer skilled PT treatment, monitor progress and collaborate with a clinical team to achieve rehabilitation goals.
The role requires an accredited PT degree, current licensure, CPR certification and reliable transportation. Pay includes base salary and pay per visit, with full benefits through Humana.
