Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their homes. You will work with a clinical team under the Clinical Manager to deliver personalized care focused on functional improvement.
Responsibilities include assessing patients, developing rehab programs, supervising PTAs and aides, documenting progress, and communicating with families and physicians.
