Job description
Home Health & Hospice Care is seeking a Licensed Physical Therapist to join our Greater Nashua homecare therapy team in Merrimack, NH. This full-time, on-site role focuses on evaluating patients in their homes, developing individualized treatment plans, and guiding patients toward independence.
You will collaborate with physicians and the homecare team, supervise PTAs, and ensure timely documentation and outcomes.
