Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to join our caring team, delivering in-home therapy to help patients regain strength and independence. You will work under a Physical Therapist to implement treatment plans, document progress, and coordinate with a multidisciplinary team in patients' homes.
The role requires a state PT license, valid driver's license, CPR, and good communication in English.
