Job description
CenterWell Home Health seeks a compassionate Home Health Physical Therapist Assistant to support patients at home under the supervision of a Physical Therapist. You will implement the treatment plan, help patients regain strength and independence, and coordinate with the care team.
This role requires state licensure, CPR, a valid driver's license, reliable transportation, and strong teamwork. You will document care weekly, participate in department in-services, and follow infection control
