Job description
CenterWell Home Health seeks a Home Health Physical Therapist Assistant to help patients regain strength and independence in their own homes under a Physical Therapist. You will implement treatment plans and monitor progress to support functional improvement.
You will administer therapeutic exercises and modalities, document records weekly, and participate in department in-services while upholding infection control and safety standards in a home-care setting.
