Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver in-home PT under the supervision of a Physical Therapist. You will implement treatment plans, document progress, and collaborate with a multi-disciplinary team to help patients regain strength and independence.
The role is full-time, 32 scheduled hours per week, with a pay range of $64,000–$87,500 annually. Requires active PT license, CPR, valid driver’s license, reliable transportation, and English fluency.
