Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will deliver individualized therapy aimed at restoring strength, mobility and independence, and document progress weekly.
Responsibilities include administering treatment plans, performing therapeutic exercises, and coordinating with the Clinical Manager.
