Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver PT treatment in patients' homes under the supervision of a Physical Therapist. You will administer exercises, therapeutic modalities, and maintain timely clinical documentation while coordinating with the care team.
Requirements include a current PT license, CPR certification, valid driver’s license with insurance, and strong team collaboration.
