Job description
CenterWell Home Health seeks a Home Health Physical Therapist Assistant to administer the physical therapy plan at patients’ homes, under a Physical Therapist’s guidance.
You will document records, participate in meetings, coordinate in-service activities, and ensure appropriate equipment and supplies are available. Requires a valid PT license, driver’s license, CPR, and strong English communication.
