Job description
CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to deliver patient-centered care in clients’ homes under the supervision of a Physical Therapist. You will participate in care planning, coordinate with the Clinical Manager, and ensure timely documentation and communication.
This is a patient-facing role with a focus on geriatric populations and functional improvement. The position requires licensure, CPR, valid driver's license, and strong teamwork.
