Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will help patients regain strength, mobility and independence through individualized therapy plans delivered in a home setting.
The role includes documenting progress, coordinating with the care team, meeting infection control standards, and participating in department in-services.
