Job description
Registered Nurse / Patient Care Manager in home health care.
Responsibilities
Deliver and coordinate high-quality in-home nursing care for patients on pharmaceutical therapies.
Conduct comprehensive assessments and develop personalized care plans.
Oversee a caseload to ensure safe and effective treatment.
Collaborate with physicians, pharmacists, and therapists to optimize patient outcomes.
Contribute to quality improvement initiatives and mentor clinical staff.
Maintain rigorous safety protocols and accurate documentation standards.
Qualifications & Skills
Registered Nurse license required.
