Job description
CenterWell Home Health, part of Humana Inc., is seeking a Home Health Registered Nurse Case Manager to deliver compassionate, in-home care for seniors and adults with chronic conditions. In this field-based role, you'll provide skilled nursing visits, complete thorough assessments, and develop individualized care plans that promote recovery and independence at home.
