Job description
CenterWell Home Health, part of Humana Inc., is seeking a Home Health Registered Nurse Case Manager to deliver compassionate, in-home care for seniors and adults with chronic conditions. In this field-based role, you’ll provide skilled nursing visits, complete thorough assessments, and develop individualized care plans that promote recovery and independence at home. You will coordinate services as the primary case manager, collaborate closely with an interdisciplinary team, and educate patients and caregivers on disease management and safety.
