Job description
CenterWell Home Health, part of Humana Inc., seeks a Home Health Baylor Registered Nurse to provide compassionate, weekend-focused in-home care to seniors and adults with chronic conditions. In this role, you will perform comprehensive assessments, deliver skilled nursing interventions, manage medications and wound care, and educate patients and caregivers to support safety and independence at home. You’ll collaborate closely with an interdisciplinary team, document care using electronic systems, and uphold high clinical and regulatory standards.
