Job description
Heartland Home Health, part of Gentiva, seeks a Home Health Registered Nurse to deliver compassionate, skilled nursing care in patients’ homes. In this field-based role, you’ll complete comprehensive assessments, manage complex conditions, administer medications and wound care, and educate patients and caregivers to support safety, recovery, and independence. You’ll collaborate closely with an interdisciplinary team, document care accurately in the EMR, and participate in case conferences.
