We are hiring a Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Carbondale, PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:

Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
/n
- /n
Perform patient assessments, medication management, and care planning
/n
- /n
Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
/n
- /n
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
/n

Work independently, but never alone

Travel to patient homes within assigned territory

RN Qualifications
/n
/n
- /n
Active Registered Nurse, RN license in the state you work
/n
- /n
One year of RN experience and the clinical expertise to deliver quality patient care
/n
- /n
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
/n
/n
/n
