Job description
Hartford Healthcare seeks a Fairfield County Revisit Nurse to provide compassionate home-based follow-up care to patients after hospital or clinic discharge. You will complete skilled nursing assessments, deliver treatments and medication management, and closely monitor patient progress to reduce readmissions.
