We are hiring aRegistered Nurse (RN)for aHome Healthposition providing skilled nursing care to patients in their homes in the Elmhurst, Bensenville and surrounding areas. This position is eligible for a $15,000total bonus potential,including a$10,000first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional$5,000sign-on bonuspaid within the first18 monthsof employment!To receive these bonuses, you must be in good standing&meetperformance expectations, and in the role and status for which you were hired!

What You'll Do:
Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
Perform patient assessments, medication management, and care planning
Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
Work independently, but never alone
Travel to patient homes within assigned territory

RN Qualifications
Active Registered Nurse, RN license in the state you work
One year of RN experience and the clinical expertise to deliver quality patient care
Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
