nWhat You Can Expect:
nThe Home Health RN Care Coordinator would work with physicians and multidisciplinary team members to develop a plan of care for assigned patients. You will also plan, implement, and document nursing care based on patient's needs. You will also make sure the patient is progressing toward desired outcomes by monitoring care through assessment and/or patient records. St. Luke's Home Health and its interdisciplinary team values integrity, compassion, accountability, respect and excellence with all the patients receiving home health. The team works collaboratively to provide quality health care to patients and families in the community.
nReliable personal transportation is required during work hours with current auto registration and auto insurance.
nHours will be between: Monday - Friday (8:30am - 5:00pm) with rotating/variable weekends including rotating/variable weekday and weekend daytime/evening call up to 8:00 pm.
nQualifications:
nnnNursing Degree
n
nnExperience: 2 years of RN experience
n
nnLicenses/Certifications: Current RN License in the State of Practice
n
nnValid Driver's License and Auto Insurance if work related travel is required for position
n
nnCurrent Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
