Job description
Cheer Home Care, LLC in San Diego is seeking an RN Care Case Manager who loves spending time with older adults and delivering high quality in-home care. You will develop and implement individualized care plans and collaborate with an interdisciplinary team to ensure seamless nursing support.
This role coordinates care among caregivers, RNs, therapists, and outside representatives, with emphasis on clinical expertise, documentation, and feedback to clients and families.
