Job description
HiredFirst in Oregon seeks a Registered Nurse (RN) - Care Manager for Home Health on a contract basis. The position features a Day shift, 8:00 AM–5:00 PM, Monday to Friday, with no routine on-call unless agency needs require it.
The RN will manage home health caseloads, coordinate care plans, perform assessments, implement care, monitor progress, educate patients and families, and ensure timely, accurate documentation while collaborating with interdisciplinary teams.
