Nascentia Health is seeking an RN Case Manager for adult homecare across Onondaga, Oneida, Oswego, Jefferson, and Cayuga Counties. You will deliver high-quality nursing care in patients’ homes, coordinate with an interdisciplinary team, and develop individualized care plans.
