Job description
UW Health is seeking a Home Health RN Case Manager in Middleton, WI. You will coordinate patient care, perform comprehensive assessments, and work with an interdisciplinary team to plan and implement patient-centered care.
An onboarding bonus of up to $5,000 is offered to eligible registered nurses. Requirements include an RN license in Wisconsin, a valid driver’s license, and at least one year of nursing experience.
