Job description
OSF HealthCare is seeking a Registered Nurse Home Care with responsibilities spanning pediatric to geriatric patients. The RN Case Manager coordinates inter-disciplinary care and delivers patient-centered services throughout the home health episode.
Qualified candidates hold active nursing licensure in Illinois or Michigan, current BLS, and a valid driver's license. A BSN is preferred; own transportation is required to support visit schedules and care delivery.
