Elara Caring is seeking a Registered Nurse (RN) Home Health Case Manager for a nursing role in North Chelmsford, Massachusetts. The position is Full Time with a $5,000 Sign-On Bonus and a Lowell territory.
As an RN Home Health Case Manager, you will coordinate and manage patient caseloads, conduct in-home assessments, develop care plans, and ensure high-quality, personalized nursing care in patients' homes. Travel within the branch coverage area is expected.
