Join Elara Caring as a Home Health RN Case Manager serving patients throughout **Boston Proper, including Chinatown**. You’ll provide skilled care in patients’ homes and help them recover, manage their health, and stay independent. That includes providing skilled nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating with physicians and other clinicians.
