Job description
Angels Care Home Health is seeking a Registered Nurse Case Manager in Green Valley, AZ to coordinate comprehensive home care and develop individualized plans of care. You will perform admission visits, evaluate eligibility, and collaborate with physicians, therapists, and social workers to ensure high-quality patient outcomes.
The role requires a current AZ RN license, valid driver’s license, and 1+ year of RN experience (2+ preferred).
