Job description
Good Samaritan Hospice, Inc. is seeking a Registered Nurse Case Manager for home health in Roanoke, VA. You will coordinate, implement, and evaluate skilled nursing care in patients' homes, guided by physician orders and individualized care plans.
The role requires 2+ years of nursing experience (home health or hospice preferred), VA RN license, BLS/CPR, and strong assessment and communication skills. OASIS experience is strongly preferred; travel and on-call responsibilities included.
