Job description
Aspirus At Home in Ironwood, MI is seeking a Registered Nurse Case Manager to join our field team. This role requires managing assessment, planning, coordination and evaluation of nursing care for Home Health patients, with partnerships across Aspirus entities and community partners.
Full-time Day Shift, 1.0 FTE, with on-call weekends; Michigan RN license and BLS required, plus valid driver's license and insurance. Prior home health or hospice experience is beneficial.
