Job description
WVUHS Home Care in Westover, West Virginia seeks a registered nurse case manager to oversee direct and indirect nursing care for patients at home. Working with the patient, family and the interdisciplinary team, you will develop, implement, and evaluate nursing care plans in accordance with agency policies.
The role involves performing home health duties, coordinating care to assure continuity, documenting in appropriate records, and remaining on-call as required.
