Job description
UT Southwestern Medical Center is seeking a Home Health RN Case Manager to provide comprehensive nursing care in the patient’s home within the Dallas-Fort Worth area. You will perform initial and ongoing assessments, coordinate services, and educate patients and families as part of the care plan.
Required RN license (TX or BNE compact), 2 years of RN experience with 1 year in adult IV care, and BLS certification. Bachelor’s degree preferred; on-site roles with a comprehensive benefits package.
