Job description
Interim HealthCare is seeking a Home Health Registered Nurse (RN) - Case Manager to provide compassionate, patient-centered care in patients' homes across CT. You will work with physicians and oversee LPNs, CNAs, and aides to deliver quality nursing services and coordinate ongoing care after hospital discharge.
You will educate patients and families on plans, medications, therapy and safety, while ensuring regulatory compliance and smooth transitions between care settings.
