Interim HealthCare is seeking a Home Health Registered Nurse (RN) - Case Manager to provide patient-centered, home-based care across multiple towns in CT. You will collaborate with physicians and oversee LPNs, CNAs and aides to deliver comprehensive care after hospital discharge.
