Job description
Amedisys is seeking a registered nurse case manager to provide high-quality home health and hospice care on a full-time daytime schedule. You will assess patients, develop care plans with the care team, and coordinate referrals to ensure comfort and quality of life for patients at home.
Responsibilities include documenting progress, teaching families, and contributing to on-call coverage as needed. A valid RN license, CPR, and driver’s license are required.
