Job description
Amedisys is seeking a Registered Nurse case manager for full-time home health coverage in Santa Clara County and surrounding areas. You will assess patients, develop care plans with the team, and supervise LPNs and HHAs. The role requires 1+ year RN experience, a valid state RN license, CPR certification, and a driving license. Mileage reimbursement is offered.
Typical hours are 8:00 a.m. to 5:00 p.m. with comprehensive benefits and a strong focus on making patient care at home the first choice
