Job description
Nascentia Health is seeking an Adult Homecare RN Case Manager to deliver high‑quality, in‑home nursing care across Onondaga, Oneida, Oswego, Jefferson and Cayuga Counties. You’ll assess, coordinate, and manage care for adults with chronic or complex medical needs, helping patients stay independent in the comfort of their homes with weekday visits and no on‑call requirement.
